Key Components of the RENEW In-Person Program

  • CDC-approved curriculum with lessons, handouts, and additional resources designed to help you make healthy and lasting changes for life.
  • Board-Certified Diabetes Lifestyle Coach support, specially trained to lead the program, helping you learn new skills while supporting you to set and meet goals.  
  • A support group of people with similar challenges and goals, who together, can share ideas, celebrate successes, provide accountability, and ultimately work to achieve success.

How does the RENEW In-Person program work?

The National Diabetes Prevention Program is based on the success of the National Institutes of Health-led Diabetes Prevention Program (DPP). Research has demonstrated that the risk for transitioning from prediabetes to type 2 diabetes can be reduced by 58% via a lifestyle program of healthy eating, regular physical activity and modest weight loss.


During the program you will receive lifestyle coaching along with support from others in the group with prediabetes to help you improve your health. You will track your food intake and physical activity, as well as participate in group discussions (if you want to) on how to overcome barriers to making healthy lifestyle choices for you.


  • 26 lessons will be delivered in-person live weekly in a virtual classroom. Lessons start off weekly and then move into a fading cycle.
  • This program offers one full year of support in total!   


What will you learn?

During the first half of the program, the focus is on:

 

  • Eating healthy without giving up the foods you love.
  • Becoming more physically active with joyful movement.
  • Managing stress by turning CALM ON with evidence based tools and techniques.
  • Dealing with challenges that can make it difficult to choose healthy food (even when you're out).
  • Staying on track toward goals and getting back on track when life knocks you off course (because life gets stormy).


In the second half of the program, you will enhance the skills you’ve learned so you can maintain the changes you’ve made. These sessions will review what's working and what you want to work on. Tools such as tracking your food, physical activity, goal setting, staying motivated, and problem solving to overcome barriers.

Do I qualify?

You can participate in the program if you are at risk for developing type 2 diabetes.

AND

  • 18 years or older
  • No previous diagnosis of type 1 or type 2 diabetes
  • Diagnosis of prediabetes within the past year or GDM (gestational diabetes) based on one or more
    • HbA1C: 5.7%-6.4%
    • Fasting plasma glucose: 100-125 mg/dL
    • 2-hour plasma glucose (after a 75 gm glucose load): 140-199 mg/dL
    • Previous diagnosis of GDM (may be self-reported)
  • Cannot be pregnant at time of enrollment



What's the cost?

Many insurance companies pay for the Diabetes Prevention Program and Behavioral Health Counseling services. For most, a referral from your doctor will be needed. 


I will provide you with a copy of your paid invoice for service that you will then submit to your insurance company for reimbursement. Information you may need for inquiry is my National Provider Number (NPI) # 1811772569

(1) The Foundation -  RENEW w/ Our Health in Harmony Lifestyle Program In-Person for diabetes prevention

  • 26 lessons delivered live virtually/in-person
  • Dedicated coach
  • One-on-One support
  • Personalized plans
  • Mindfulness Based Stress Reduction (MBSR) tools that are proven to work
  • Investment for you $697

 *HSA, FSA, Medicare (no cost to you if eligible), and Self Pay 


(2) Enhanced Support -  RENEW w/ Our Health in Harmony Lifestyle Program for diabetes prevention 

+ 1:1 Coaching via Zoom for an additional 16 private sessions (50 min. each) of one-to-one coaching support custom tailored to you. Topics may include any common coaching topics such as:  

  • Health & Nourishing Your Body
  • Work/Career & Effective Life Balance
  • Restorative Rest & Play
  • Love & Relationships
  • Purpose & Aligned Living  
  • Investment for you $1,897

 *HSA, FSA, and Self Pay 


New groups are anticipated to start on a monthly basis.
